How is this different from a nurse paralegal review?

Nurse paralegals summarize the chart. We verify the chart — running the audit trail, checking for late entries, backdating, deletions, and producing an independent authenticity finding before the clinical analysis even begins.

Do you provide expert witness testimony?

We provide written declarations and affidavits on records authenticity and audit-trail findings. For medical expert testimony on standards of care, we work alongside retained clinical experts and can refer them when needed.

How long does an analysis take?

Two to three weeks is typical for a focused chart. Complex matters, multiple providers, or large date ranges take longer. Rush turnarounds available on a case-by-case basis.

Is the engagement HIPAA-compliant?

Yes. A Business Associate Agreement (BAA) is available on request, file transfer is end-to-end encrypted, and records are retained per your engagement letter and destroyed on close.

What records do you need?

The complete legal EMR export, audit logs, access logs, and amendment history. We provide jurisdiction-aware request language that producers cannot easily push back on.

What if the audit trail is missing or incomplete?

That is itself a finding. We document gaps, the discovery requests that should produce them, and the inferences a finder of fact can draw under your jurisdiction.
